DOCTORvsVIRUS (read as Doctor AND Virus, not Doctor VS Virus - don't get it wrong!) is a solo card game you can play with any regular deck of playing cards with two jokers.
Drop cards to make sets of PILLs to treat your patient, but be careful not to make any VIRUSes! Also, don't take too long, or your patient might not make it...

So I am a bit confused the instructions show cards flipped and flipped over cards are worth 3 points instead of 1. But when do you flip cards over and in what case
The cards are never flipped on the playing field! I just show them as flipped in the instructions to try to make it clear that those cards aren't relevant to the specific instructions getting explained in that rule (didn't want to add more visual noise), but I see now why that might be confusing.
You only flip the face-up pills to be face-down if you want to reshuffle your hand.

I like how your deck of cards has both Aces and ones (under "Numbers are in this order"). That might have been an oversight on your part, but I actually have decks like that (although they'd then be more than 54 cards).
EDIT: Also, isn't it weird to gain two points when you use a pill for a reshuffle rather than lose them?
That is an oversight! It should be only A - I've updated the instruction manual to reflect that.
Also, fewer points is better (lower score is good!) so that's why you gain points when you use a pill.
So the goal is to make the fewest pills that let you win? That seems... counterintuitive.
Making fewer pills is actually harder than making more pills, and it means you have fewer pills to use for reshuffles, so it's an additional challeng to make the game harder if you want.
